Hello Tom,
I've not seen this form before, I don't think... The fall colors of the photo are just gorgeous... and the words go so well... with both photo and form...
I wondered at the third 'quiet' which reads:
[Quite] streets,
as cars on the byways
saunter down the roads of renown,
(should this be quiet?)

Thank you Cat. I considered the saunter as a very slow cruise. But I did misspell quiet. It just my dyslexia.

This sure is a nicer scene than the ice storm we got in Nebraaska on Thanksgiving - your snow is even better than ice.
